All participants of trips or events with Malibu Presbyterian Church are required to fill out a Medical Release Form. For your convenience, we offer two options:- An Online Medical Release, which will generate a pdf and email it to you. It is a safe, secure and easy way to keep a copy of the release for yourself.
- Standard blank release to be downloaded and filled in by hand.
